Expression of Matrix Metalloproteinase in Kawasaki Disease

Abstract
Purpose: In Kawasaki disease(KD), there could be cardiovascular complications, including coronary aneurysm or myocarditis. Many studies reported matrix metalloproteinase(MMP) might be contributed in these complications. We investigated relationship between MMP and cardiovascular complications in KD.

Methods: Heparinized bloods were obtained from children with KD treated with intravenous immunoglobulin(IVIG) in acute and subacute phase. We measured expression of MMP-2, MMP-8, MMP-9, and TIMP(Tissue inhibitor of MMP) by RT-PCR(reverse transcriptase polymerase chain reaction) and zymography, and compared levels of MMPs between acute and subacute phase. Total 27 patients were divided two groups: group 1 was defined as children without cardiac complications, group 2 with complications.

Results: Nine children among 27 patients had coronary aneurysm(4 patients) and myocarditis(5 patients). The total leukocyte counts, neutrophils, and C-reactive protein significantly decreased after IVIG administration. The levels of MMP-2 in RT-PCR significantly increased after IVIG(P<0.05) and in zymography were different pattern in both groups between before and after treatment, as decreased in group 1 and increased in group 2(P<0.05). The level of MMP-8 increased in group 1 after IVIG, but decreased in group 2(P<0.05). The expressions of MMP-9 were not significantly different after treatment. The ratios of MMP-2/TIMP decreased in group 1 and increased in group 2 without significant difference.

Conclusion: Our results suggested MMP-2 and MMP-8 may be contributed cardiovascular complications in KD, and increased level of MMP-8 in acute phase. Finally, we concluded MMP-8 may be used as a new marker for cardiovascular complications in KD.
